Write down the date,
the agency, the person you talked to, and the information exchanged.
Contact the Washington County Sheriff’s
Office to File a Report
Have your bank account and credit card
account information available. Prepare the information concerning
your identity being used by another person and the loss to you or
your bank / credit card.
Contact Each Bank and Each Credit Card
Company
If your checks and or your credit card
(s) have been stolen or used fraudulently: Notify each bank, each
card company, and each business involved and offer to complete and
sign affidavits of forgery as needed.

Notify the three
credit reporting bureaus to report the loss and to put a Fraud
Alert on your account (s).

Freeze your credit

Contact The FEDERAL TRADE COMMISSION To File A Fraud Affidavit
Call the Federal Trade
Commission Identity Theft Hotline to notify them and get advice on
how to proceed 1-800-438-4338.
Contact the Federal
Trade Commission on the internet for information and to download
affidavit forms. This affidavit can be copied and sent to all
banks, financial institutions, merchants, and companies involved.
This affidavit can be notarized.

To report a missing
driver’s license and apply for a new driver’s license as soon as
possible. Check to see if anyone has applied for a license in your
